Terms & self-certification

DC ABCA patient registration — terms

By submitting this registration you are applying for a DC medical cannabis patient registration with the DC Alcoholic Beverage and Cannabis Administration (ABCA). You confirm that:

  • You are 21 or older (18–20 year olds cannot self-certify and need a healthcare provider's recommendation).
  • All information you provided is true, accurate, and matches your government ID.
  • DC residents: your registration needs a DC home address; if your ID doesn't show it, ABCA accepts proof such as a lease, a utility/bank statement (last 2 months), a recent pay stub, voter card, DC vehicle registration, or property deed.
  • Out-of-state visitors: you are applying for a DC medical cannabis patient registration valid for the period you choose, which requires a government photo ID and a registration fee.
  • Green Legacy uses your information and ID photo to create your customer profile and prepare/submit your ABCA registration.

Self-certification statement

By checking the self-certification box you certify that:

  • You will only use cannabis purchased from a licensed District of Columbia retailer;
  • It will be used for a qualifying medical or dental condition, or the side effects of a qualifying treatment;
  • The information is true and correct, certified under penalty of perjury — a false statement is punishable under DC law by up to $1,000, 180 days, or both.
